全身振动训练联合体外冲击波对早中期膝骨关节炎患者的疗效研究

目的:本研究旨在探讨体外冲击波联合全身振动训练对早中期膝骨关节炎患者的临床疗效。方法:以早中期膝骨关节炎患者为研究对象,采用随机数字表法分为对照组(n=39)与实验组(n=37)。对照组仅接受体外冲击波治疗,实验组接受全身振动训练联合体外冲击波治疗,治疗周期为8周。采用WOMAC评分、VAS疼痛评分、Lysholm膝关节评分、膝关节活动度和炎症及基质金属蛋白酶作为结局指标,分别于治疗前、治疗8周后进行评估,并在治疗12、20周随访。结果:两组患者治疗后及12周随访时各项指标均较治疗前显著改善,且实验组在各项指标改善程度方面均显著优于对照组。20周随访时,对照组除膝关节活动度较治疗前差异不显著外,其余指标均显著改善。结论:对于早中期膝骨关节炎患者,全身振动训练联合体外冲击波治疗较单一的体外冲击波治疗在减轻患者疼痛、改善膝关节功能、提高膝关节活动度及调控炎症水平方面具有更显著的协同效应和临床优势。

Abstract:

Objective: This study aims to investigate the clinical efficacy of extracorporeal shock wave therapy combined with whole-body vibration training in patients with early-to-mid stage knee osteoarthritis. Methods: Patients with early-to-mid stage knee osteoarthritis were selected as experimental subjects and randomly divided into a control group(n=39) and an experimental group(n=37) using a random number table method. The control group received only extracorporeal shock wave therapy, while the experimental group received whole-body vibration training combined with extracorporeal shock wave therapy. The treatment period lasted eight weeks. Outcome measures included WOMAC scores, VAS pain scores, Lysholm knee scores, knee joint range of motion, and levels of inflammatory and matrix metalloproteinases. Assessments were conducted before treatment, after 8 weeks of treatment, and at 12-and 20-week follow-ups.Results: Both groups showed significant improvements in all outcome measures after treatment and at the 12-week follow-up compared to baseline. The experimental group demonstrated superior improvements in all indicators compared to the control group. At the 20-week follow-up, all indicators except knee range of motion in the control group showed significant improvements compared to baseline, while the difference in knee range of motion in the control group was not statistically significant. Conclusions: For patients with early-to-mid stage knee osteoarthritis, whole-body vibration training combined with extracorporeal shock wave therapy exhibits more significant synergistic effects and clinical advantages over extracorporeal shock wave therapy alone in alleviating pain, improving knee joint function, enhancing knee joint range of motion, and regulating inflammatory levels.
